11/30/2023 EnchantaVaughn Approved 1.Behavior: QMHP-C greeted Laylah upon arrival to the home setting. Upon arrival to the home setting, Laylah appeared to be in a positive mood evident by her facial expressions such as a smile on the face. QMHP-C engaged Laylah in a discussion to determine any new behaviors since the last session. Laylah verbalized to QMHP-C, “No not really just my attitude Ms. Vaughn”. According to mother behaviors last about 3-4 minutes lasting 30 minutes in duration. 2. Behavior: Laylah participated in the session and did not need any redirection from QMHP-C. Laylah was able to provide QMHP with major consequences set by her mother when she presents negative behaviors. At times she became frustrated with her mother (mother not present) when recognizing her negative behaviors. According to mother behaviors last about 3-4 minutes lasting 30 minutes in duration.. 1. Intervention: QMHP-C began the session by inquiring about Laylah’s current thoughts, feelings and emotions. QMHP-C informed Laylah that the session would focus improving her attention span as well as reducing impulsive behaviors due to Laylah exhibiting impulsive behavior patterns and receiving negative consequences that have resulted from it. QMHP-C modeled active listening and asked open-ended questions in order to have Laylah speak in greater detail. Through the use of modeling, role-playing, and behavior rehearsal QMHP-C modeled to Laylah how to stop, think, and plan before acting. 2. Intervention: QMHP-C had Laylah to make up her own role-playing scenarios of impulsive behaviors to reenact. QMHP-C encouraged Laylah to review the use of “stop, look, listen, think, and plan” in day to day living and identify positive consequences. QMHP-C assisted Laylah in making a list of consequences of impulsive behaviors. Due to Laylah interaction with others lately in the school arena, QMHP-C modeled and taught Laylah social skills (i.e. calm tones, considering the impact of her comments, etc.) that control impulsivity. 1. Response: Laylah appeared to be receptive to QMHP-Cs’ interventions and services that were provided to her during the session. Laylah admitted that she tends to just act without thinking about what might happen when she makes inappropriate decisions. In the beginning of the role-playing, Laylah did take it too seriously due to her laughing at QMHP-C. Laylah was laughing, making jokes, leaving in and out of the session, etc. However; after she was given the opportunity to make up her own role-playing scenarios, she tended to take them more seriously. 2. Response: Laylah was able to list several consequences of impulsive behaviors. Some of these included: not getting her hair done, being suspended from school, having in school suspension, not being allowed to go over her friend’s house. Laylah didn’t believe that she displays inappropriate behavior in school when it comes to being unacceptably too loud. QMHP-C Interpretation of client’s progress towards today’s objectives: Laylah continues to struggle with communicating her feelings, thoughts, and emotions in an appropriate manner. QMHP-C Plan of Action: VACCD staff will continue to assist Laylah with being able to control her anger in an appropriate manner as well as ways to assist with conflict resolution.
